OUR LEGAL DUTY
Illinois Women’s Health & Wellness (“IWHW”) is required by law to maintain the privacy of your protected health information (“PHI”), provide you with this Notice of Privacy Practices, and follow the terms of this Notice.
HOW WE MAY USE AND DISCLOSE YOUR PHI
Treatment
We may use and share your PHI to provide, coordinate, or manage your healthcare. This includes sharing information with other healthcare professionals involved in your care, including consulting clinicians when appropriate.
Payment
We may use your PHI for billing and payment activities. IWHW is a cash-based practice and does not bill insurance for visits.
Healthcare Operations
We may use PHI for practice operations such as quality improvement, training, compliance, and administrative purposes.
Business Associates
We may share PHI with third-party service providers (such as telehealth platforms or transcription services) who perform services on our behalf. These entities are required by law and contract to protect your PHI.
Consulting Clinicians
With your authorization, PHI may be shared with collaborating physicians or consultants for treatment guidance, patient safety, or quality assurance. Such consultation does not establish a provider-patient relationship with the consultant.
Required by Law
We may disclose PHI when required by federal or Illinois law, including public health reporting, abuse or neglect reporting, or legal proceedings.
TELEHEALTH & ELECTRONIC COMMUNICATIONS
IWHW provides telehealth services. Your PHI may be transmitted electronically through secure systems for intake, messaging, prescribing, and documentation. While safeguards are used, no system can guarantee absolute security.

You have the right to:

COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with IWHW or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
